Health declaration for Certificate of Fitness (CoE).

If you need a Certificate of Fitness (VvG) for a driving license, you fill in a Health Declaration: a form with questions about your mental and physical health which you send to the CBR. Based on the submitted Health Declaration, you will receive from the CBR the standard examination report which - for elderly and professional drivers - must always be completed by a doctor, but also any other references. You can receive this by mail, but preferably CBR continues the process digitally. This shortens the processing time considerably. You will receive a message from the CBR when the Certificate of Fitness has been registered. Without this declaration you cannot apply for a driving license.

Medical examination

To apply for a driver's license from 75 years of age and for driving license categories C1, C, C1E, CE, D1, D, D1E and DE, you must always undergo a medical examination. This may also be the case if you answered "yes" to one or more questions on the Health Declaration or if you have a disability or impairment. There is a separate Health Declaration for each of these situations.

Requesting Health Declaration

This can be done in three ways

  1. online with your DigiD via My CBRexternal link icon
  2. At driving schools affiliated with the CBR
  3. at the municipality's front desk. To do so, you must make an appointment online or by calling 0492-378 500.

At the front desk, you can purchase the form for yourself or someone else.

How long does it take?

You will be given the Health Declaration directly at the counter. You must fill it out and send it to the CBR. You will receive notification from the CBR about your Health Declaration. On the website of the CBRexternal-link-icon you will find more information.
